MEMBERSHIP APPLICATION
All membership applications for allied health professionals and health coaches shall be filed using a prescribed application form course through and passed upon by the membership committee of and finally approved by majority vote of the Board of Trustees of the International Federation of Lifestyle Medicine Professionals.
Criteria for Allied Health/Health Coaches Membership
Membership Requirements to submit together with application form:
Retention as active members
General Procedure:
To remain as active members, they must fulfill CME credit hours earned as reviewed by Committee on Membership Credentials; and must have been updated in payment of their annual membership dues on or before December 31 of each year. Evaluation and Accreditation on or before February 28 of each year. The concerned member will be informed of her/his status in March of each year.
Active Membership:
Criteria for Allied Health/Health Coaches Membership
- Filipino
- Lifestyle Medicine Trainee
- Has good moral character, and has not been charged/ convicted of any moral turpitude, criminal offense.

- 2 copies of 2 x 2 recent picture
- Payment of application (P1750)
Retention as active members
General Procedure:
To remain as active members, they must fulfill CME credit hours earned as reviewed by Committee on Membership Credentials; and must have been updated in payment of their annual membership dues on or before December 31 of each year. Evaluation and Accreditation on or before February 28 of each year. The concerned member will be informed of her/his status in March of each year.
Active Membership:
- Updated payment of annual dues (P1750).
- Earned the required CME credit hours, complied according to the table below:

He / She must attend at least 1 annual or midyear convention every year and must earn atleast 15 PCLM CME units within 3 years. |
Certified Member |
He/She must attend at least 1 annual or midyear convention each year and must attend Post-graduate course. A PCLM Diplomate must earn at least 60 PCLM CME credit units yearly to maintain Certification status. |
Note: CME credits can be earned from PCLM/IFLMP activities such as webinars, conferences, post-graduate course, round table discussion, speaking assignment, active committee engagement with project implementation, research and other related activities.
