MEMBERSHIP APPLICATION
All applications for membership shall be filed using a prescribed application form course through and passed upon by the Committee on Membership Credentials, Evaluation and Accreditation and finally approved by majority vote of the Board of Directors.
Criteria for Membership
Membership Requirements to submit together with application form:
Retention as active members
General Procedure:
To remain as active members, they must fulfill CME credit hours earned as reviewed by Committee on Membership Credentials; and must have been updated in payment of their annual membership dues on or before December 31 applicable for the next calendar year. The evaluation is conducted on or before February 28 of each year. The renewing member will be informed of her/his status in March of each year.
Active Membership: Open to any Filipino physician who is/has:

- Filipino physician
- Active member of the Philippine Medical Association
- Updated and authenticated license from the Professional Regulation Commission
- Lifestyle Medicine Trainee
- Has good moral character, and has not been charged/ convicted of any moral turpitude, criminal offense.
Membership Requirements to submit together with application form:
- Photocopy of updated and authenticated PRC card (back to back)
- Photocopy of updated and authenticated PMA card (back to back)
- Practice credentials (residency/diplomate/fellowship certificate)
- Other practice modality certifications (e.g. functional medicine, integrative medicine, etc.)
- 2 copies of 2 x 2 recent picture
- Payment of application (P1750)

- Licensed to practice medicine by the Professional Regulation Commission Board of Medicine (updated)
- Member of good standing of the Philippine Medical Association (updated).
- Updated payment of annual dues (P1750).
- Summary of LM practice portfolio covering LM competencies and advocacy.
- Earned the required PCLM CME credit hours, complied according to the table below:

He / She must attend at least 1 annual or midyear convention every year and must earn at least 60 PCLM CME units annually. |
Diplomate |
He/She must attend at least 1 annual or midyear convention each year and must attend Post-graduate course. A PCLM Diplomate must earn at least 60 PCLM CME credit units yearly to maintain Certification status. |
Fellow |
He/She must attend at least 1 annual or midyear convention each year and must attend Post-graduate course. A PCLM Fellow must earn a total of 90 PCLM CME credit units yearly. |
Note: PCLM CME units can be earned from PCLM activities such as webinars, conferences, post-graduate course, round table discussion, PCLM-related speaking assignment, active committee engagement with project implementation, research and other related activities.
